What Happens During Toric IOL Surgery

The folded toric IOL is then inserted through the small opening and unfolds inside the lens capsule. The full procedure typically takes around 20 to 30 minutes per eye.

Once the IOL is positioned inside the eye, your Cataract Surgeon rotates it to the planned alignment axis, which corresponds to the steepest curve of your cornea. The small marks on the lens edge act as visual guides during this step. Careful alignment planning done before surgery, combined with precise surgical technique, gives the best chance of achieving the target correction.

You remain awake during surgery but relaxed. A mild sedative is often offered to ease nerves, and numbing drops keep the eye comfortable throughout. Most patients describe their visual experience during the procedure as a soft blur of light and color rather than anything detailed or startling. Recovery After Toric IOL Surgery

A standard monofocal IOL restores focus at one distance but does not correct astigmatism. Patients who have significant astigmatism and receive a standard lens will likely still need glasses for distance vision after surgery. A toric monofocal IOL addresses both the cataract and the astigmatism in a single step, which is why it is often the preferred choice for patients with meaningful levels of astigmatism.

Limbal relaxing incisions (LRIs) are small arc-shaped cuts made at the outer edge of the cornea during cataract surgery that reduce low levels of astigmatism by gently reshaping the corneal curve. LRIs are generally suited to mild astigmatism only and provide a less predictable result than a toric IOL for moderate or higher levels. Some premium lenses combine astigmatism correction with extended or multiple focal points, allowing patients to see more clearly at a range of distances rather than just one. A toric IOL corrects astigmatism and targets clear vision at one distance, most commonly distance. It does not correct presbyopia, which is the age-related loss of near focusing ability that makes reading glasses necessary for most adults over 45. Most toric IOL patients still use reading glasses for small print and close-up tasks. If reducing the need for reading glasses is also a priority, a toric EDOF or toric multifocal lens may be worth discussing with your Cataract Surgeon.

If rotation does occur, it is most likely to happen in the very early healing period, which is one reason the one-week follow-up visit is important. A minor surgical procedure can reposition the lens if it has rotated enough to meaningfully affect your vision.

Intraocular lenses are designed to remain in the eye permanently. A toric IOL does not wear out or need to be replaced under normal circumstances. Once properly placed and stabilized, it is expected to provide consistent vision correction for the rest of your life. Routine annual eye exams are still important for monitoring overall eye health and catching any unrelated changes early.

Posterior capsule opacification (PCO) is a clouding of the thin lens capsule behind the IOL that can develop months or years after any cataract surgery, including toric IOL procedures. PCO causes a gradual return of blurred or hazy vision that can feel similar to the original cataract. It is very common and completely treatable.
